1. VOID WHERE PROHIBITED. The Chevy All-Star Salute to Special Olympics Illinois Raffle (“Raffle”) is a random drawing raffle; it is sponsored by and all net proceeds benefit Special Olympics Illinois (“Sponsor”), 605 East Willow Street, Normal, IL 61761-2682. Participation constitutes each entrant’s full and unconditional acceptance of and agreement to be bound by these Official Rules.
2. Eligibility To Enter: The Raffle is open to licensed drivers at least 18 years of age who are legal residents of all counties in Illinois, Kenosha and Walworth counties in Wisconsin and Lake, Porter, La Porte, Newton and Jasper counties in Indiana (unless prohibited). Employees of Sponsor are not eligible to enter. To enter, eligible entrants must purchase one or more raffle tickets at the price of $20 per ticket or $30 for 2 tickets during promotional period beginning September 3rd, 2009 thru November 1, 2009, or while supplies last. At the time of entry, ticket purchasers choose from among the following Chevrolet Celebrity Spokespersons (“Celebrity”): Dan Hampton, Ron Santo, Lou Piniella, Ozzie Guillen, Patrick Kane or Jonathan Toews. Tickets may be purchased online at www.allstarsalute.com or www.ChevyDrivesChicago.com,at promotional events, at select Chicagoland and Northwest Indiana Chevy dealers or by calling 888-23CHEVY. Completed ticket form and payment may be mailed to Special Olympics Illinois, 800 Roosevelt Rd., Bldg B-220, Glen Ellyn, IL 60157 (“Sponsor Northern Office”) and must be postmarked by October 31, 2009 and received by November 5, 2009 to be considered eligible. Odds of winning depend on number of eligible entries received; no more than 10,000 tickets will be sold.
3.Selection of Winner: On November 6, 2009, a grand prize drawing will be held at Sponsor Northern Office. One (1) winner will be drawn from all eligible entries. Winner need not be present at the grand prize drawing to win.
4. Notification: Sponsor will attempt to notify winner based on contact information provided in entry. Entrants are solely responsible for providing Sponsor with accurate and updated information. Winner must acknowledge acceptance of their prize to Sponsor within 72 hours of Sponsor’s attempted notification, and may be required to complete and return within such time period an affidavit of eligibility and a liability and publicity release. Winner may not assign its right to receive the prize. If, for any reason, the potential Grand Prize winner is ineligible, unable or unwilling to take delivery of the vehicle or alternate cash prize, then the potential winner will be disqualified and forfeit the prize and an alternate winner may be selected by Sponsor in Sponsor’s sole discretion at random from among all remaining entries; in such event, the disqualified potential Grand Prize winner will not be awarded any prize.
5. Prizes: Grand Prize: One (1) 2010 Chevrolet Equinox courtesy of the Chicagoland and Northwest Indiana Chevrolet Dealers, retail value $25,975. Grand Prize winner must a have valid U.S. driver’s license and proof of insurance to take delivery. Vehicle may differ from prize shown in promotional materials and may not be transferred or assigned. Grand prize winner may elect to receive alternate cash prize of $10,000 in lieu of vehicle.
The Grand Prize winner will also receive four (4) tickets to a Chicago Blackhawks home game on November 13, 2009, 4 Tickets ARV ($240). Tickets will be assigned by Chicagoland and Northwest Indiana Chevrolet Dealers, and may not be substituted or exchanged; A single item autographed by their chosen Celebrity, ARV ($75) and a meet and greet with at least one of the Chevy Celebrities before, during or after the November 13, 2009 Chicago Blackhawks game.6. Sponsor assumes no liability or responsibility in connection with the safety or performance of prize awarded, or in connection with any warranty or representation therewith. Grand Prize winner must personally retrieve and take delivery of vehicle by December 31, 2009 from a participating Chicagoland and Northwest Indiana Chevy dealership at a time and date to be determined by the Winner with approval of the Sponsor. Grand Prize winner is responsible for all taxes (federal, state, local) and fees (including title, license, insurance and vehicle registration fees, and for all costs associated with any available upgrade/option packages and any other expenses related to the acceptance and use of the prize not specified herein. Grand Prize winner must supply taxpayer identification number (Social Security number) prior to receipt of the Grand Prize. Sponsor must receive tax installment prior to delivery of Grand Prize (checks to be made payable to Special Olympics Illinois), and winner will be issued IRS forms for value of prize.
7. General Terms: By participating in this Raffle, entrants agree to be bound by these Rules and all decisions of Sponsor, which shall be binding and final. Sponsor and its affiliated entities are not responsible for printing, distribution, production, or other errors that may affect the Raffle, its operation, or its integrity, and may rescind, cancel, or revoke this Raffle based on such error and without liability by refunding all tickets purchased. Entry constitutes permission to use Winner’s name and likeness in any and all media or venues now or hereafter known in perpetuity for advertising and/or promotional purposes without further remuneration (except where prohibited by law). Sponsor, its affiliated entities, and Sponsor personnel are not responsible for incorrect, incomplete, or illegible contact information provided by entrants. Each entrant, including each winner, agrees to release, indemnify and hold harmless Sponsor and the Chicagoland and Northwest Indiana Chevrolet Dealer Group, their respective parents, subsidiaries and affiliates and each of their respective employees, representatives and agents, as well as the delivering dealer, (collectively, the “Released Parties”) from and against any liability, claims, actions, losses, or damages arising in any way from participation in or administration of this Raffle or acceptance, failure to accept, receipt, ownership, use, or misuse of any prize. To the extent that some provisions may not be applicable in whole or in part, the liability of the Released Parties shall be limited to the fullest maximum extent permitted by law, including, where permitted, eliminated altogether. Sponsor respects your privacy, and entrants’ information will not be used except by Sponsor, the Chicagoland and Northwest Indiana Chevrolet Dealer Group and the delivering dealer and as otherwise provided in these Rules. Logos are registered trademarks of Sponsor and the Chicagoland and Northwest Indiana Chevrolet Dealer Group and use is prohibited without express consent.
8. Winners List: To obtain a copy of the winners’ name, send your request in writing along with a stamped, self-addressed envelope to: Special Olympics Illinois, Attn: Chevrolet Equinox Winner List, 800 Roosevelt Road, Bldg B-220, Glen Ellyn, IL 60137. Requests must be received by December 6, 2009.
